Insurance Billing For Therapeutic Services With Sara McRae, LMT
Pre-Authorization is required before billing insurance for your therapeutic services with Sara McRae.

Welcome to the insurance billing page for therapeutic massage services with Sara McRae. If you have Pacific Source insurance and work for the city as a first responder or city employee, we are here to assist you in getting your massage therapy covered. If you have other insurance you may also be eligible. Please read the following information carefully to ensure a smooth process:

1. Pre-Authorization and Verification:
Before billing your insurance for therapeutic massage services, pre-authorization is required. Some insurance companies may have specific authorization requirements or deductibles that need to be met before you can receive benefits. It is your responsibility to verify your insurance benefits for therapy to understand your coverage.
To verify insurance coverage, please submit your insurance information to our billing agent before booking your first appointment. Our billing agent will confirm your coverage and copay amount.

2. Required Information:
When submitting your insurance inquiry, make sure to include the following information:
- Mention that the insurance inquiry is for a session with Sara McRae
- Full Name
- Date of Birth
- Phone Number
- Full Address
- Email
- Insurance Company
- Insurance Company Phone Number
- Insurance Policy/Group Number
- Your Insurance ID or Claim Number
- Employer Name
- Date of Injury or Accident (if applicable)

3. Payment and Reimbursement:
If your insurance coverage has not been pre-approved before your appointment, full payment will be due at the time of service. We can provide you with a receipt that you can submit to your insurance company for direct reimbursement. My rates reflect a $45 discount for payment at the time of service. Your insurance will be billed at the listed rate, plus $45.

4. Workers Compensation or Motor Vehicle Accident Insurance:
Please note that Workers Compensation or Motor Vehicle Accident insurance claims usually only cover one-hour sessions.
